  • A brief description of the issue: To provide the diagnosis from the owner or a professional.
  • Images and videos: These will be required when the issue relates to a mechanical or workmanship defect. (Example: ā€œThere is an issue with my faucet color finishā€).
  • Shipping Information: Confirm full shipping address information including your phone number and apartment number if applicable.
